The South Carolina Department of Health and Human Services has awarded the Medicaid Division of the Celerian Group a Long Term Care (LTC) Eligibility Assistance contract. The purpose of the contract is to establish and operate a liaison center to assist nursing home providers and their applicants with the Medicaid Long Term Care eligibility process, including creation of application assistance materials, providing application assistance and enrollment status updates, resolving application issues and facilitating submission of completed applications for SCDHHS review and LTC eligibility determination.
